Sustainability Training: Transition Plan for Healthy Start Projects

The goal of this Sustainability Training was to provide support to grantees as they completed the Transition for Healthy Start Projects form. During this webinar, participants also heard from grantees who have been successful in developing and implementing sustainability plans for their HS projects.
