Case Managers and Care Coordinators

Overview

Case Managers and Care Coordinators are critical members of the Healthy Start (HS) team, working directly with individuals and families. They develop a strength-based, collaborative relationship with HS participants and their families to help meet their health and social needs. Case Managers and Care Coordinators may see participants virtually or in-person, and in their homes, HS project office, or other settings.

Case Management & Care Coordination Services

Case management/care coordination includes the following services:

  • Screening and intake
  • Comprehensive assessment of needs
  • Creation of service care plan
  • Identification of appropriate services
  • Linkages to additional services
  • Monitoring of progress
  • Reassessment as needed
  • Creation of discharge plan and case closure
