NACCHO Workshop Series: Tools for Community Transformation Workshop #4: Community Health Improvement Matrix

During this workshop, participants were introduced to the Community Health Improvement Matrix, a tool developed by NACCHO to evaluate and develop strategies for a community health improvement plan. The CHIM helps communities identify strategies that address social determinants of health and systemic drivers of inequity.
