On behalf of the Division of Healthy Start & Perinatal Services (DHSPS), the Healthy Start TA & Support Center (TASC) is hosting the Community Consortium Plan Kick-off Webinar on Wednesday, September 18 from 2-3:30 pm ET. As part of the 2024-2029 grant cycle, Healthy Start grantees must collaborate with their Community Consortium to create a community-driven action plan to address the factors and conditions beyond clinical care that contribute to disparities in perinatal outcomes. These plans should be based upon results of a community needs assessment and environmental scan that identifies and prioritizes SDOH causes of disparities in perinatal outcomes in the project area, and should describe strategic partnerships and strategies to address the “upstream factors” and unique social and structural determinants of health (SDOH) contributing to disparities in perinatal outcomes within the project area.
During this webinar, TASC and DHSPS staff will discuss the NOFO requirements for the Community Consortium plan and share available resources and support to help your program in the planning process. Consortia leaders and Project Directors are encouraged to attend this webinar. Each project may also invite up to two consortia members.
